Embrace All Latino Voices addresses chronic disease within the Latin community of Mecklenburg County.  The program implements monthly educational events and seminars focused on chronic disease prevention and management. (Heart disease, diabetes, and obesity offering preventive measures, treatment options, and lifestyle changes that can be adopted.

Harmony Health Therapeutic Services delivers tobacco prevention programming to support adolescents' youth of color.  including vaping education and awareness, mental health resources, and tobacco recovery groups.

Harvesting Humanity deploys culturally responsive community health workers to support a “Fitness For All” program facilitating individual and group health plans, including the Health is Wealth Nutrition program and cooking classes for various age and ability levels, Yoga, and Mindful Movement Exercises.

Home4Me

Home4Me implements (3) 30-day Intergenerational Health Challenges, offering education and workshop sessions to youth and adults. Weekly workshops on meal planning, cooking demonstrations, and information on the benefits of physical activity.

IC Puerto Nuevo

IC Puerto Nuevo addresses Chronic Disease prevention and management services in the Latino community by offering chronic disease education and providing participants with equipment/supplies such as blood pressure monitors, scales, measuring cups, etc. They facilitate fair health events such as stroke scans, mammogram tests, and blood pressure checks.  IC Puerto Nuevo also provides a Quick Reaction Food Security Program, providing residents with packaged meals and food items and the expansion of our community garden project, "Terra Nova" in Partnership with Steel Creek YMCA.

Kika’s House provides bi-weekly health workshops, group fitness classes, and peer education for children ages eight and older.

Learning Help Centers provides the African American and Latino population with health education, implementing a healthy lifestyle, diabetes prevention & mental health training along with health assessments & medical interventions.

The Queens Collective

The Queens Collective focuses on Mothers, Fathers, & Babies to ha learn where food comes from and how to incorporate a healthier diet during pregnancy and into their family's overall diet. Participants also e receive prevention-based tools and resources for addressing mental health and navigating perinatal challenges.

 

Trips for Kids Charlotte

Trips For Kids Charlotte provides a series of educational and youth cycling programs for the Fall and Spring Program Seasons.  They also provide a series of community ride programs in the fall focusing on the North Druid Hills, Lakeview, and Winterfield communities.  Trips for Kids facilitates youth rides at the Southview Recreation Center and Arbor Glen Education, as well as popup mechanical clinics ("Free Bike Repair") Programs.

Wheatfield Ministry Inc. provides activities including Zumba classes (both aerobic and chair-based) available in both in-person and virtual formats, Double-Dutch aerobics, forming accountable walking groups, and offering community workshops on healthy cooking and meal preparation.

Young Explorers Enrichment Center

Young Explorers Enrichment Center offers a comprehensive "Living Fit For Life" program, which significantly emphasizes physical activity, nutrition, and educational initiatives for the entire family. Young Explorers also offers a School/Non-School Day Program (October 2023-June 2024), which includes healthy cooking classes, demonstrations, and nutritious recipes tailored for children and parents/caregivers.

Church on the Grow hosts monthly health education and physical activity sessions, like dance classes/lessons, pickleball, golf lessons, and provides healthy snacks and health resources to participants.

Factors of the Seven facilitates a weekly feeding program for underserved residents within Charlotte’s Optimist Park community.

First Baptist Church West sponsors physical activity like line dancing sessions, monthly senior day parties, as well as blood drives.  First Baptist Church West also provides a healthy eating program called the Grab & Go Lunch Program, which provides service to adults aged 50-59 who do qualify for senior meal programs.

Greater Bethel AME Church

Greater Bethel AME Church provides hybrid (Online and in-person) healthy cooking lessons and physical activities that address obesity, diabetes, and heart health for youth and adults. Greater Bethel also offers Individual smoking cessation sessions to provide information on the hazards of smoking, smoking dependency, and what triggers smoking.

Greater Salem Church

Greater Salem Church provides training and certification for Community Health Workers and Caregivers.  They Offer training in CPR, First Aid, and Mental Health First Aid and Health Navigator/Ambassador programs.  The program is a one-stop shop concept for families dealing with diabetes and their caretakers. Their Villager program for single persons living with diabetes includes extended support through on-site or virtual educational sessions and training workshops (12 Sessions), biometric testing, caregiver resources and supportive tools, home visits, and resources provided by community health workers and caregivers. The Heights Ministries

The Heights Ministries provides healthy and nutritional food options for the community. Their program addresses food scarcity by providing access to nutritional meals year-round via our community garden, food pantry, and summer feeding program. The Heights Ministries will host various workshops and trainings on chronic disease prevention and nutritional meal preparation.
