Live Well Meck connects Mecklenburg County Public Health with community partners to regularly collect and review local health data. This helps us understand the health status of residents, community needs, strengths, and challenges.

Through the Community Health Assessment (CHA) process, Live Well Meck identifies where our community is thriving and where improvements are needed. The CHA relies on input from diverse partners and residents through surveys, focus groups, forums, and advisory committees, ensuring community voices shape the work.  This information guides priorities, funding decisions, and policy changes to support the health and well-being of everyone in Mecklenburg County.

The CHA process is ongoing. After each report, we develop the Community Health Improvement Plan (CHIP) to address the priorities identified in the CHA. Progress on these goals is shared annually through the State of the County Health (SOTCH) report.

MECKtrics- How we check our progress

MECKtrics describe Live Well Meck’s set of 14 core community health indicators. These indicators show how our entire county is doing on the most important factors that affect health and quality of life.

  • 14 Core Indicators: MECKtrics includes measures tied to CHA priority areas including access to care, chronic disease, mental health, maternal and child health, and violence prevention.
  • Population View: Instead of looking at one person at a time, MECKtrics reflects the health of the whole community. This helps us see where we are strong and where we need to focus more effort.
  • Trends Over Time: We track these 14 indicators each year to see whether our community is improving, staying the same, or facing new challenges.
  • Transparent and Accessible: Progress on these indicators is shared annually in the State of the County Health (SOTCH) report, and up-to-date information is available through a public scorecard.

With MECKtrics, Live Well Meck makes complex health data clear and easy to understand giving everyone a snapshot of our community’s health and a way to track progress together.
