Programs focus on chronic disease prevention and address tobacco prevention/control, food security, and active living through policy, systems, and environmental change strategies.

Office of Chronic Disease Policy & Prevention Action Plan FY24-28

Making Healthy Choices Easier is the Office of Chronic Disease Policy & Prevention action plan for fiscal year (FY) 2024-2028. The plan builds on data and priorities of the 2022 Mecklenburg County Community Health Assessment and the actions identified in the FY2023-2025 Public Health Strategic Business Plan. Our plan explores the two leading causes of death in our community: heart disease and cancer. We also considered chronic conditions, risk factors, and behaviors that drive the incidence of heart disease and cancer. This action plan outlines the policy, systems, and environmental changes our team will lead in order to make healthy choices easier in Mecklenburg County.
