
Care Management for High Risk Pregnancies (CMHRP)
The Care Management for High-Risk Pregnancies (CMHRP) program provides care management services for high-risk pregnant women. Care managers help pregnant women connect with resources to promote healthy outcomes for mom and baby for up to two months after delivery.
Your Care Manager can help you by...
- Providing accurate information and tips to assist you with following your providers’ treatment plan.
- Coordinating your care across all your providers.
- Linking you with needed services and referrals to meet medical and social
- needs.
- Referring you to other programs, like childbirth or breastfeeding education classes, family planning, and the Women, Infants and Children (WIC) nutrition program.
- Offering information and tips to take care of yourself during pregnancy and postpartum.
- Helping you manage your medications.
- Meeting with you after you have been in the hospital or the emergency room.
- Assisting you with follow-up appointments and answering questions

Eligibility
Care management services are available to individuals who are pregnant or within 60 days postpartum and are enrolled in North Carolina Medicaid. In Mecklenburg County, a limited number of uninsured individuals may also receive this service.
