HIV Case Management

The HIV Case Management Program functions to assist individuals 18 years of age and older with HIV to link services to better maintain medical stability. We can link you to healthcare and psychosocial services to ensure timely and appropriate level of care. Case managers provide a range of client-centered services that such as education, testing, counseling and case management for HIV & sexually transmitted diseases. 

Eligibility Requirements

Clients must meet criteria in the following areas: residency, income, diagnosis, photo ID, and a willingness to comply with the program guidelines.

Cost and Payment Information

There is no cost to clients for HIV Case Management services.
