Community Alternative Program for Disabled Adults

The Community Alternative Program for Disabled Adults (CAP/DA) is a Medicaid Home and Community-Based Services Waiver authorized under section 1915 (c) of the Social Security Act. The program allows elderly and disabled adults, ages 18 and up, to receive support services in their own home as a safe alternative to nursing home placement.

  • Skilled nursing visits
  • Adult day health care
  • CAP In-home aide or personal care aide services
  • Durable medical equipmentHome delivered meals
  • Respite services
  • Personal emergency response services
  • Specialized medical supplies

You can apply for the Community Alternative Program by phone or completing an application.

Call the Mecklenburg County Health Department CAP/DA Intake Line at 704-336-4674,
and request a referral be submitted on your behalf.
 

Call NC LIFTSS (the state agency) at 833-522-5429 and request that a referral be
submitted on your behalf.
